My husband has been a resident at Coal Creek for the. Past 2 years. Initially he was in assisted living and now he is in long term skilled nursing. I am visiting the facility several times each day. I find the staff to be knowledgeable and caring. It is a clean and cheerful building. The activities for residents are well planned. My husband likes the food and is well taken care of. The administration is quick to resolve any issues that arise.

After an extensive total hip revision i was sent here. Nightmare!!
There are a few, (very few) people who are great.
I am literally still having panic attacks from my time there. I was traumatized and abused.
PLEASE DON'T SEND A LOVED ONE TO THIS FACILITY.
I left AMA but I felt my hip was in danger of being totally messed up from the lack of experience and training of the staff.
I would rate it lower than a one if I could.

I want to share my heart felt thanks to Coal Creek. My mom was transferred here 52 days ago. She had advancing dementia and she had lost the ability to walk. We had hoped that she could rebuild her strength and come back to be cared for at home. After a few weeks it came clear she would need more care then I could give her she moved to long term care. In all her time her she was always up, dressed, clean, and out of her room when I came to see her each day. Even when my mom was being very difficult she was always treated with kindness, respect, and dignity. I felt so comfortable that I was able to take respite. My mother is at the end of her journey, the care and compassion shown not just to my mom, but to my family has been humbling. I will forever be grateful that moms last days where here at Coal Creek.

I do NOT recommend this facility they just leave the patients in the room they say they do physical therapy but it’s a lie even my dad said he was only walked once a week if that otherwise they left him in bed. They can say they do physical therapy but it’s shows they don’t
Twice I went in to visit his air hose was laying on the floor the breakfast and lunch tray were still on his table
I don’t think they change the sheets unless you say something
There was pee on the floor for days
He said they didn’t give him one shower the whole time he was there said they just used a sponge
I know his health was not good but they were just there to collect money
This place is a mess I see why they changed their name
Please look around other places before you send your loved one here

This facility and providers lie. Said a elderly man was at 75% mobility when in fact then never got him out of bed. I 100% feel they transfered him because his benefits were running out.
Then to top off the situation they transfer him on the wrong O2 amount causing a hypoxic episode requiring hospitalization. I have reported them and the provideres to the state.

Run fas and run far from these elderly abusing people.

We had a good experience here after my elderly Mother had a fall and smacked her little head. Good Sam sent her for a stay after anesthesia complicated her recovery. The nurses were so patient with Mom's mental decline and unfortunate drooling. Every time we had a question Nina was easy to reach. We are quite thankful.

My father had a minor stroke and a seizure; after several days in the hospital he needed to go to a rehab center. After many arguments and negotiations, he gave in and went to Coal Creek Post and Assisted Living Facility. I looked at another facility before choosing Coal Creek, and I was horrified. That place had pretty good reviews, but I wouldn’t have left my worst enemy there.
When I went for my tour, I knew Coal Creek would be the right place for my dad, the minute I walked in the door. It felt both safe and beautiful.
When we arrived, the caring, smiling face of the woman at the front desk and warm welcome by the head of admissions, made us all feel important there.
Dad’s room was spacious and had a small refrigerator. The bathroom was large and looked new. It felt like one of those basic hotel rooms that we all know.
The food was good, and the nutritionist cared not only about the patient’s dietary restrictions, but what they really liked. There would be extra ice cream if she knew it was your favorite! The kitchen staff was also very friendly.
The nursing staff does a good job, but keep in mind, this is not a hospital. They will not be checking on you or your loved one nearly as often and they do their best with the limited staff that they have. These are usually “travel nurses” so they do not work full-time and are new to the facility in general. I’m sure that is the new normal everywhere. Post pandemic. There is a call button to page the nurse.
The PT and OT therapists were good. They both were knowledgeable, compassionate, and skilled. I only wish that there were more of it. Your time doing PT and OT is limited.
The weekends are very quiet with a smaller staff, and no hustle and bustle. That would be a good time to do lots of visiting.
I highly recommend Coal Creek Post and Assisted Living Facility. They did a good job for my dad.

AVOID this place unless you have 24/7 to babysit and make sure what needs to happen happens! And DON'T be deceived by the beautiful atrium. It's gorgeous indeed but that's the END of the road for anything of quality, competence and care. The rehab rooms have phones that don't even work and my dad often waited over an hour for a call button to be answered. He was left on the toilet for almost an hour because the CNA never returned to help him and no one answered when he pulled the emergency cord. Same thing happened when he was given a nebulizer breathing treatment. The CNA put the mask on him, started the machine, said she'd be back in 10 minutes and NEVER returned! My dad ended up turning the machine off himself and then it sat idle on his bedside for 3 days when he was supposed to be receiving the treatment twice a day. I could go on for days with concrete examples. My dad ultimately developed open wound bed sores from such neglect that we pulled him out and transferred him to an Assisted Living Community that gives way better care than the "Post Acute Rehab" at Coal Creek! Sadly, there ARE a few FANTASTIC individuals who work here, though I'm sure not for long. Unfortunately, these rock stars are few and far between and can't carry a facility that is fundamentally broken and riddled with complacency and clueless management! I inquired early on about several safety concerns and was told that the Administrator, Carly Bader, would reach out to me. NEVER happened! When I finally confronted her after 3 separate people told me she would be calling me, she claims she was calling a wrong number. Even after that, she still just hid in her office and ignored all of my concerns, never once following up with me.
Run while your loved one is still alive as they may not be much longer if you send them here!
